Vendor Profile

Vendors interested in registering their company for upcoming opportunities may register by completing a Vendor Profile Form. To fill out a form:

  1. Click on the ​​CMS MWSBE Vendor Profile form on the Documents page. The form will open as a fillable PDF document in your internet browser.
  2. Use the tab key to navigate from one question to the next.
  3. Enter your data on the corresponding line provided. Please note that some profiles may require attachments (i.e. certification certificate).
  4. Once the form is completed, save it to your desktop and email the completed Vendor Profile and related documents (if applicable) as an attachment to one of the contacts on the form.  You will be notified once your application is processed.
