Athletics
- Middle school and high school athletes must complete the CMS athletic participation form and return to the school in order to participate in athletics.
- High school athletes need to also complete the NCHSAA Preparticipation exam form as soon as possible.
- Middle school athletes have a $50 participation fee per sport season. Fees will be collected on a per sport basis.
- High school athletes have a $100 participation fee per sport season. Fees will be collected on a per sport basis.
- All athletic participation fees can be online with a debit or credit card or a check or money order at the school the student attends.

Child Nutrition
- Meal benefits end on Oct.7 for those students that have not submitted a new application this school year. Federal Guidelines require that students who were approved for meal benefits last year must reapply for benefits between Aug. 1 and Oct.7. Applications are available at your child’s school, you can call the Child Nutrition office and request one be mailed to your home or online at /cmsdepartments/cns/Pages/documents.aspx.
- Students that have been eating meals free or at a reduced price since the start of school that have not submitted a new application by October 7 will be charged the full price of $1.25 for breakfast and $2.05 for lunch beginning October 8. Parent/Guardian can get help with completing meal a application by calling (980)343-6041 or stopping by the Child Nutrition office at 3301 Stafford Dr., Charlotte, NC 28208 between 7:30-4:30 Monday-Friday.
- Applications for new students and for students whose families have a change in circumstances may be made at any time during the school year.
- Effective Sept. 12, 2011 students approved for reduced price meals will begin receiving breakfast at no charge. The NC Legislature passed a law this summer that provides state funding to subsidize the reduced price breakfast. A nutritious breakfast is served daily in every CMS school.
- Payment for past due invoices for 2010-11 meal charges must be made no later than Sept. 30, 2011. Payment may be made by check or cash to the cafeteria manager at your child’s school or by debit or credit card at www.paypams.com After Sep. 30, all balances due will turned over to the Finance Department for review.
Coordinated School Health
- Students entering Pre-K, kindergarten, sixth grade, as well as new students to CMS need to be in compliance with state requirements for immunizations.
- Physicals exams /health assessments for Pre-K and kindergarten students only are also needed.
- It is the parent’s responsibility to provide the information on the first day of school. If not, the law allows 30 calendar days for the parent to submit the appropriate certified document of immunizations and/or a physical completed within 12 months prior to school entry.

- The 30th calendar day is Sept. 23 and any child who does not meet the requirement will be excluded from school by the principal. This means that the student would not be permitted to attend school on Monday, Sept. 26.
