Course Facilitation Documents

The documents below are to be used by Family Advocates and Parent University Liaisons that are hosting events within their school.

• Course Overview Form - this form is to be used to turn in all evaluations, sign in sheets, registration forms from completed course.
• Course Registration Form & Evaluation Form (English & Spanish) - these forms are to be used during all classes.  The information requested is kept confidential and is used for accountability and for the funding purposes of the program.
