Former Student Records Request Information

  • Per CMS policy and the Family Educational Rights and Privacy Act (FERPA) student records are only available to those who have authorization from the student or parent, guardian or responsible person if the student is less than 18 years of age.
  • If you are 18 years of age or older, school records will not be released to a parent, guardian, or responsible person without your written consent. Please complete the Consent to Release Student Records form and email to or, bring the completed form to 4335 Stuart Andrew Blvd for pick up.

Charlotte-Mecklenburg Schools will be operating on a four day work schedule from Monday, June 15th to Friday, July 31st.  Normal operating hours during this period will be Monday through Thursday, 7:30am to 6:00pm.

  • There is a $10 cash fee per certified transcript to obtain  a state ID, passport, birth certificate or tax verification.

Online Orders at:

  • CMS offers an online secure and convenient way to order transcriptsThere is a $10 fee for each order plus a $4 convenience fee for credit and debit card payments.
  • There is a $40 processing fee for the request of a student's complete CMS cumulative record. Please allow up to four (4) weeks for processing.
  • Photo ID is required for pick up orders. 
  • Records will only be released to the individual that the records belong to or to the parent or legal guardian of students under the age of 18.
  • Upon submission of your request, CMS Student Records department will retrieve and mail the record(s) to the address(es) designated on your form.
  • Once mailed, your payment will be processed and you will receive an email confirmation and receipt. 
  • Phone and fax requests will not be accepted due to security reasons.

  • We do not provide copies of your SAT/ACT scores. For copies, contact the test provider or contact The College Board at
